SERVICE AGREEMENT
Clark Limousine Service (referred to as "Provider") agrees:
- To provide service as scheduled.
- There is not liability in the event of a mechanical breakdown while on charter and will only be responsible for providing client time loss at a mutually agreeable date and time.
- There is no liability for delays due to in climate weather or other "acts of God".
- To terminate trip without refund if the clauses in this agreement are breached and/or if any laws are broken.
- There is no liability for items left in vehicles.
Client agrees:
- No smoking or vaping in vehicles.
- Once service is booked, no refunds are allowed however, provider will consider rescheduling service within 90 days of original service date.
- To be responsible for all broken or missing items from the vehicle, including glasses.
- To accept full financial liability for any damages to the vehicle by themselves or anyone in their party.
- To pay a cleaning fee of a minimum of $250 for body fluids, food, spills etc.
- To pay any fees, tolls and of fines incurred by the provider if directed by the client.
- No food in vehicles.
- No decorations or writing on vehicle without written approval from Jeffrey Clark, owner.
- They are at least 21 years of age or a parent/guardian must complete a liability release form.
- No illegal substances or weapons permitted in vehicle.
- Private partition will be used by/for adults only.
- Additional hours are charged 15 minutes after scheduled trip termination and will be charged to the card on file.
